Thank you for applying to our family retreat program! We have received your application form and will begin the selection process shortly.
- Please remember to email or fax us a letter from the participant’s healthcare provider on professional letterhead and include the applicant’s current diagnosis, plus any physical, emotional, or cognitive delays, medications, and allergies. The letter must be signed and dated, including a professional title, the healthcare facility’s address, phone number, and email. (participants applying for the Veterans, LGBTQ, or Bereavement retreat are exempt from this basic eligibility requirement).
- All eligible applications will be entered into a participant lottery, and selected families will receive notification within 3-business days of the application deadline by a Deliver the Dream representative.
- A completed application form does not guarantee selection for acceptance into the family retreat program
