Thank you for your interest in applying for a Family Retreat. A completed application does not guarantee acceptance into the Family Retreat program. All eligible applications will be entered into an applicant lottery system to select the final family retreat participants. Participants are notified by Deliver the Dream within 3-business days of the retreat application deadline (typically a month before the retreat).
We also need a letter from a healthcare professional stating the applicants current diagnosis plus any physical, emotional or cognitive delays. It should include all their medications and allergies. The letter should be on letterhead and include their professional title, work address, phone, email, and be signed and dated (excluding the veterans and bereavement retreat).
